Medical History Form<\/h4>\n

All new international students are required to complete a New Student Medical History form, which can be found in the Admission portal.<\/p>\n

Physical Exam<\/h4>\n

The physical exam portion must be completed no earlier than one year prior to your arrival to LVC. The physical exam is a good opportunity to review all required immunizations with your doctor.<\/p>\n

Immunizations<\/h4>\n

Immunization information must be completed and immunizations up-to-date in order for your health record to be considered complete. Some majors (i.e. Physical Therapy) may require additional entrance requirements. Please inquire directly with the Clinical Education coordinator or your advisor about requirements.<\/p>\n
